Privacy & product information

Updated September 17, 2026. This page describes the current beta.

Information you choose to provide

FORM stores your account identity and the information you enter: profile details, body measurements, exercise and nutrition records, goals, preferences, habit reflections, recovery activities and optional progress photos. Optional fields can be left blank.

These records support your personal workspace, history, estimates and recommendations. Your workspace is not a public profile.

Your progress photos are private

Within FORM, your photos are only available through your signed-in account. Other users cannot view them, and connecting with a coach does not give that coach access to your photos.

Photo requests are checked against your account. Photos are not published to a public gallery or automatically attached to feedback emails. You can delete individual photos whenever you choose.

FORM does not sell your training records or use advertising trackers.

Coaches Corner and sharing

Coaches keep separate client profiles, assessments and workout plans. Connecting your personal account requires your approval. Connecting does not share personal records by default. You choose workout completion, set details, weight and body-fat entries, daily habit check-offs, cardio, selected goals, recovery and pain reports, and nutrition summaries or a detailed diary. Date limits control which records are included. A sharing preview shows the same filtered records available to your coach. Progress photos and private habit reflections are not shared. You can stop personal sharing while continuing to receive workouts. Permission changes apply to new requests immediately. Already viewed or copied information cannot be recalled.

You can disconnect in Coaches Corner at any time to stop future access. Coach-entered assessments and programming remain in the coach’s workspace; contact your coach to request their removal. Deleting a coach account removes its client workspaces and assignments. Deleting a personal account stops its sharing. Workouts you already accepted remain in your own saved plans. Email notifications contain a sign-in link; delivered correspondence cannot be recalled.

Feedback and access

When you submit feedback, your message, the page you identify and your account email are emailed to FORM’s owner. Your workout records and photos are not automatically attached. Submission identifiers and timestamps are retained for delivery retries and abuse prevention.

Exporting and deleting your information

You can export your complete saved app records from Account settings. Photos must be downloaded separately before deletion. Individual records can be edited or removed where the relevant page offers those controls.

Account deletion removes your active saved app records and uploaded photos. For email accounts, it also removes your Supabase sign-in account. For accounts using an external sign-in provider, deletion removes FORM records, not your account with that provider.

A minimal account identifier and deletion timestamp remain to prevent old sessions from restoring deleted records. Deletion across services can require a retry if a provider is unavailable. Provider backups and security logs may persist under their retention practices. Previously emailed feedback remains in the recipient’s mailbox and provider email records. You can request removal of that correspondence separately.

What FORM’s recommendations mean

FORM is a planning and tracking tool for adults. Its recommendations use programmed rules and estimates. The app has not been clinically validated to diagnose, treat or prevent a medical condition, and it does not guarantee strength, weight-loss or other outcomes.

Calorie requirements, body-fat entries, estimated one-repetition maximums and muscle contribution values have limitations. Estimated training contributions are planning weights, not exact measurements of biological stimulus. The anatomy viewer and exercise library are educational references, not a complete clinical atlas.

Pain-related filters can help you review movement demands. They do not identify an injury or prescribe rehabilitation. Supplement information describes evidence and limitations, rather than an individual prescription. Evidence confidence is specific to the claim and is not a guarantee of benefit or safety for every person.

You decide whether to accept recommendations. Use appropriate professional advice for medical concerns or individual restrictions. FORM and its feedback inbox are not emergency services.

More about storage, service providers and access

Authorized service operators may need to access stored information for support, maintenance or security. Account privacy does not mean that nobody operating the service can ever access stored files.

Supabase handles email sign-in and password authentication. The website runs on OpenAI Sites infrastructure using Cloudflare services. Saved app records use a database, and uploaded photos use separate object storage. Resend handles account and feedback email. Food searches send your search terms to USDA FoodData Central.

These providers process information needed to deliver their services, including technical request and security logs. Processing may occur outside your country. FORM does not promise that records remain in Canada.

Sign-in cookies keep your session active. Device preferences, such as your theme, may be stored in your browser. Pending edits and the last workout set edit can be kept on this device so you can recover after an interruption. FORM checks your signed-in account before reopening these drafts. Pending copies clear after synchronization when no undo is retained. Clearing browser data before synchronization removes these local drafts. Account deletion clears drafts on the device used to delete the account. The current app does not include advertising trackers or sell your training records.
